Below is a wiring diagram for an HSH setup using an 4 pole super switch and a master volume and master tone control. This setup uses 500k pots for both volume and tone and a 0.022uF capacitor.
The super switch enables coil splitting in positions 2 and 4 to give those familiar "in between" single coil positions you get on a standard Strat type setup.

The below version uses a standard single wafer blade switch (CRL or Oak Grigsby) and achieves the same - auto splitting in positions 2 and 4 (both are hum cancelling)
